right now for sse it's servername, tool name, and description, and for stdio it's tool name and description - but still figuring out what makes sense here, so far I'm mostly concerned about the description changing i.e. "malicious comments" (also technically you wouldn't get the code from a running server) - but I'm not super clear on server names if there is utility there, and also AFAIK you can have duplicate tool names...
